Instructions to Complete Your Application

Your completed application packet should contain:

  1. The information as required by the questions that follow these instructions.
  2. One letter of recommendation, from an instructor. The recommendations should evaluate specifically your entrepreneurial qualities. The letter should be submitted in a sealed and signed envelope.
    Fellowship recipients will be asked to provide a second letter of recommendation from a former employer. Recipients may request that MBA Admissions release one of their letters of recommendation for admission in place of the recommendation from a former employer.

Submit all materials in a single 9" x 12" envelope. They may be sent by US post or private carrier, or dropped off at the receptionist's desk at F451 Haas School. Retain a copy of your responses.

Application

Keep a copy of all answers. No application materials or copies will be returned by the Center to any applicant.
